Weight in Pregnancy Best Controlled by Diet, Study Suggests

ScienceDaily (May 18, 2012) — Pregnant women, including those who are obese or overweight, should be encouraged to minimise weight gain through diet, according to major new research from Queen Mary, University of London.

Piling on excess weight during pregnancy increases the risk of complications for pregnant women but doctors have been cautious in advising women on ways to manage weight for fear of any adverse effect on mother or baby.

However, the new study published in the BMJ shows that following a healthy diet, overseen by health professionals, stems excess weight gain in pregnancy and reduces the risk of pregnancy complications such as pre-eclampsia, diabetes, high blood pressure and early delivery.

Half the UK population are either overweight or obese and the rates are rising. Around a third of women gain more than the recommended amount during pregnancy.

Previous research has linked obesity during pregnancy with an increase in a variety of risks including high blood pressure, diabetes, miscarriage, birth defects, blood clots, pre-eclampsia, and even maternal and infant deaths.

The new research, which brings together the results of 44 separate studies, is the largest of its kind and includes data on more than 7,000 women. It was commissioned and funded by the NIHR’s Health Technology Assessment programme.

The researchers investigated the effect of diet, exercise, or a combination of the two. They looked at how much weight women gained throughout pregnancy and whether mother or child suffered from any complications.

Although all three methods reduced the mother’s weight gain, diet had the greatest effect with an average reduction of nearly four kilograms. Exercise only resulted in an average reduction in weight gain of just 0.7kg. A combination of diet and exercise only produced and average reduction of one kilogram.

Women who followed a calorie controlled diet were 33 per cent less likely to develop pre-eclampsia, one of the most dangerous pregnancy complications that presents with raised blood pressure and protein in the urine. their risk of gestational diabetes was 60 per cent lower, their risk of gestational high blood pressure was 70 per cent lower and their risk of early delivery was 32 per cent lower. However, the researchers acknowledge that these findings need to be confirmed by further large studies.

Crucially, babies’ birth weights were not affected by dieting.

The research was led by Dr Shakila Thangaratinam, a Clinical Senior Lecturer and Consultant Obstetrician at Barts and the London Medical School, part of Queen Mary, University of London with researchers in UK and Europe. she said: "We are seeing more and more women who gain excess weight when they are pregnant and we know these women and their babies are at increased risk of complications."

"Weight control is difficult but this study shows that by carefully advising women on weight management methods, especially diet, we can reduce weight gain during pregnancy. It also shows that following a controlled diet has the potential to reduce the risk of a number of pregnancy complications.

"Women may be concerned that dieting during pregnancy could have a negative impact on their babies. this research is reassuring because it showed that dieting is safe and that the baby’s weight isn’t affected."

Dietary advice was based on limiting overall calorie intake; balancing protein, carbohydrate and fat; and eating foods such as whole grains, fruits, vegetables and pulses.

Dr Thangaratinam added: "what we don’t know is why diet should be so much better than exercise in controlling weight gain. It could be that it is simpler and easier for women to stick to. It may also be that eating a high-fibre diet has other positive health effects for a pregnant woman."

Combining data on the thousands of women who participated in these trials will also allow researchers to further examine the effects of diet and exercise across women of various ages, body mass index, ethnicity, socioeconomic status and medical conditions.

The Women’s Health Research Unit at Queen Mary, University of London has recently established an international collaboration on Weight Management in Pregnancy (i-WIP) to answer these questions.

Women are extremely complex creatures – from their emotions, to their mental thoughts, and to their physical bodies, the species is not anything less than complicated.  one of the hardest areas for women is how the physical body ages, and whether there is a realization of it or not, the reality is, for many, evidence of age for women comes with her inability to bear children.  For some reason, that looming cloud not only signifies that childbearing years are over, it seems to indicate another harsh truth, youth is slipping away. that is why getting pregnant over 40 seems more complicated than it should.

What is that magical, or, more of the looming number that hovers around women waiting to descend and tell them fertility is no longer a possibility?  unfortunately, it begins around 35, is when the eggs seem to be fewer and few, making it harder for women to conceive – and although it is not fair, given that men can produce children well into their seventies and eighties – it is a reality that women must face.

But that is not to say that hope is lost, on the contrary.  Women over 40 deliver healthy babies safely everyday.  there are some things to remember, however, we know enough about women’s health today to help women over 35, 40 even 45, have safe pregnancies and produce healthy babies.  Conception after 35 is difficult because a woman does not ovulate as she once did – often, there may be months when there is no egg, making the process of conception become more scientific.

Regardless of your age, remaining healthy and in shape will help with conception.  Women that are over 35 have enough challenges; they should not be compounded with other issues like weight, high blood pressure, high cholesterol, and other ailments that can be easily treated through natural healthcare.  By having a routine medical exam, you can see the areas of health that need to be improved, and, have the doctor examine you thoroughly to see if there are any prevailing issues that may make conception more difficult.  Begin exercises, maintain a good diet and take vitamin supplements to prepare your body and make it ready for conception.

Age for many is a number, for women trying to conceive the number is something that is always in the back of their minds.  But women need to remember, conception and pregnancy are possible with age – it may take longer and more effort may be involved, but getting pregnant over 40 is not even close to being impossible.

Like some of you reading this post, I learned I was pregnant with twins right around Week Six. A few days after I had taken a pregnancy blood test, my doctor’s office called asking me to come back in for an ultrasound as my test had an “elevated level of hCG.” I panicked but the nurse assured me that there was no need for worry. “We just want to see if you miscalculated the date of your last period or if you’re pregnant with twins,” she said.

As I was always one of those women who marked the day my period arrived on a calendar, I knew something was up. I tried to warn my husband that we could in fact be expecting twins but he balked at the suggestion. His attitude soon changed to utter surprise and shock for there on the ultrasound screen were two little flickering heart beats. Twins.

If you get called in for a transvaginal ultrasound early on in your pregnancy, you too might not see much on the screen but rest assured that by Week Six (a mere four weeks after conception) the brains, kidneys, livers and spines of your twins are developing rapidly although each baby is barely larger than the size of a grain of rice. by Week Eight, your babies have hands, feet and even elbows, too. Fingers are beginning to form.

It’s also between Week Six and Eight that you may begin to experience some pregnancy symptoms, you know, those pesky problems that plague all pregnant women during the first trimester but unfortunately are heightened for moms carrying twins due to a higher concentration of hormones. you may notice, for instance, that you’re getting tired way more easily. That’s perfectly normal as your body is busy at work nurturing two growing babies. in fact, you should slow down and take it easy! Start getting into the habit of taking frequent rests preferably lying down on your left side as that will increase the blood flow (and nutrients) to your babies.

You may notice a marked increase in your appetite, too. Don’t fight it! Although a mom-to-be expecting a singleton requires about 2,300 calories a day, mothers pregnant with twins need a minimum of 2,700 calories a day (3,100 is optimal). Give into your cravings (mine were hamburgers) and load up on those calories with special attention paid to protein. In a multiple pregnancy, protein is king as research has shown that protein puts the weight on babies and helps prevent preterm labor and premature delivery. And the earlier the weight gain the better as it aids in the development and function of the placenta. Aim for a weight gain of 20 pounds by 20 weeks.

Week Six to Eight Pregnancy Tips:

  • If you suspect you’re pregnant, start taking a daily multivitamin until you get to the doctor’s office where she’ll write out a prescription for prenatal vitamins. however, make sure your over-the-counter substitute contains enough folic acid, known to help reduce the risk of neural tube birth defects such as spina bifida.
  • To help fight morning sickness, don’t let yourself get too hungry for that’s when symptoms exasperate. instead, try to keep a constant flow of food going all day. Many small snacks rather than three large meals tend to curb the feelings of nausea. As strange as it sounds, keep food with you at all times–a snack in your handbag or briefcase, one in your car, and even one in your office.
  • Slow down! Cut out strenuous exercising such as aerobics, cycling or hiking, for instance. Less activity means fewer burned calories leaving more for your growing babies. take frequent rests during the day, too, putting your feet up, or if you can, lying on your left side. Shoot for three mini-naps every day.
  • Think about visiting a perinatologist, or maternal-fetal medicine (MFM) specialist, an obstetrician who specializes in high-risk pregnancies. (And, yes, if you’re carrying twins you’re considered “high risk” regardless of your age.) you can use a perinatologist as either your primary-care physician or as a consultant. Either way, it’s great to have an extra set of eyes trained to spot potential problems.

At one point near the end of her pregnancy she was placed on bed rest and had to miss several episodes of Desperate Housewives. Crucial scenes requiring her presence were actually filmed at her house in California.

In 2009, Sarah Jessica Parker, 47, used a surrogate to have twin girls Marion and Tabitha with actor husband Matthew Broderick.

The Sex and The City star had been married for 12 years when her daughters were born.

Canadian pop star Celine Dion went through IVF in 2000 to have her son Rene Charles and used it again in 2010 to have her twin sons, Nelson and Eddy.

The 44-year-old confessed that she had a miscarriage and had six rounds of IVF before her successful pregnancy.

After IVF, some couples get pregnant without help

Despite the label of being “infertile,” some couples who have tried fertility treatments are later able to have a baby naturally, according to a new study from France.

In some instances from the research, the parents had had another child previously using in vitro fertilization (IVF) — while in other cases the couple had a baby even after an unsuccessful experience with IVF.

“Most infertile couples think that they are unable to conceive spontaneously whereas our study shows (this) remains possible,” Dr. Pénélope Troude at the French national medical research institute, INSERM, wrote in an email to Reuters Health.

“Our results should give hope to couples who have been unsuccessfully treated by IVF,” Troude and her colleagues wrote in their report, published in the journal Fertility and Sterility.

Researchers have previously reported that couples waiting for IVF treatment will occasionally drop off of the wait list because they become pregnant without fertility treatment — what doctors refer to as a “spontaneous” pregnancy.

To get a better sense of how frequently people going through IVF end up having babies without extra help, Troude and her colleagues collected information on about 2,100 couples who had begun fertility treatment in France in the early 2000s.

About 1,300 of those couples successfully ended up having a baby through IVF.

Eight to 10 years later, the couples responded to a survey about whether they’d had a child on their own following fertility treatment.

Among the parents who’d had a baby through IVF, 17 percent later had another child without assistance. And among couples who originally failed to have a baby with fertility treatment, 24 percent went on to have one from a spontaneous pregnancy.

“It must be borne in mind that infertility did not mean no chance to conceive but low or very low chance to conceive,” Troude said.

Dr. Johannes Evers, a professor of obstetrics and gynecology at Maastricht University Medical Center in the Netherlands, said that couples’ behavior can explain why people whose IVF didn’t work out had a higher rate of natural pregnancies afterward.

“Successful couples already had their child(ren), so they will have used contraception,” Evers, who was not involved in the study, wrote in an email to Reuters Health. Men and women who were younger had a better chance of having a baby naturally, as did couples whose infertility didn’t have a clear cause.

For instance, among women younger than 35 with unexplained infertility, 45 percent became pregnant after failing to have a baby through IVF.

Infertility can be caused by hormonal problems or low sperm count, for example, but in 12 to 13 percent of couples in the study, the cause was unknown.

Troude said unexplained infertility could be a good sign for couples’ chances of having a baby, compared to those who have a clear reason for not initially getting pregnant.

Evers said the findings “reassure (couples) that it is not the end not to conceive by IVF, especially not if they have unexplained infertility.”

Another recent study found that among couples who hadn’t been able to get pregnant after a year or more of trying, 44 percent of those who opted against fertility treatment still ended up having a baby eventually (see Reuters Health story of February 2, 2012).

Still, it would be difficult to use the new findings to determine which couples might benefit from a wait-and-see approach, and which couples should proceed with IVF, Troude said.

IVF runs for about $15,000 a cycle, and may or may not be covered by insurance.

Troude added that the decision is made all the more challenging given that as a woman ages, her chances of conceiving decline.

Although the findings offer some hope for couples trying to have a baby, she said the long follow-up time in the study and relatively low pregnancy rates “correspond to a very low conception probability.”

Evers said the numbers might also overestimate the true birth rate.

That’s because only a little more than half of the couples who were invited to participate in the study actually answered the questionnaire, and “pregnant couples will have been more likely to answer than disappointed, childless couples,” Evers said.

Mother reclaims life for two sons with help of CLIMB Wyoming

When the call came, she was in the hallway of her apartment, getting her son dressed.

Khara Ledoux had tried not to get her hopes up. she told friends how competitive it was to get into the program. Thirty-two women had applied.

“They only take 10 girls,” Khara had said.

She listened to the program director on the other end of the line and screamed.

Khara wanted a better life for her two sons.

She had used methamphetamine. she was on food stamps. her boys’ dad called her a welfare mom.

The phone call meant a new start: no one would be able to call her that anymore.

Even when she was using, Khara thought meth was trashy.

She grew up in Louisiana, in a nurturing home with two sisters and a brother. she never hung out with people who did drugs.

The first time she saw someone shoot up, she felt like she was in a movie.

“I was just like, ‘Oh, my God, I can’t believe I’m watching this,’” said Khara, now 33.

“And then it just became a norm.”

Khara followed a boyfriend to Casper 12 years ago, when she was 20. she met friend Liz Davis serving at Red Lobster. In the summer of 2004, they started using.

Khara’s mom had died earlier that year. Even though they lived far away, Khara and her mom had been close and talked on the phone all the time.

Two years later, her sister Joy died, too.

“It was just one thing after another,” Khara said.

Khara didn’t feel responsible to anyone. she used because she felt like she wasn’t hurting anybody.

“It’s such a hard thing to stay away from. It’s easy to get in a rut,” she said. “… my life was going nowhere anyway.”

Some nights Khara only made $15 or $20 in tips waiting tables. she and Liz arrived at work late. a dishwasher once told them he could see their track marks.

Khara got evicted. she and Liz spent one winter living out of their cars in parking garages around Casper. Liz remembers cranking the heat up to get the car nice and steamy, then shutting it off and settling in for the night.

In Texas, Khara’s sister Babette Curtis worried. she would call Khara again and again and hear nothing back. she called a nurse in Casper to ask about meth use. If Khara was her sister, the nurse said, she would drive up and take her away.

Babette planned to come to Casper, then a winter storm hit. The roads were too poor for travel.

“The thought of losing another sister, I couldn’t bear it,” Babette said.

Khara had talked about leaving Casper to go live with her sister, too. she told Liz often she was going to get away someday.

Liz didn’t think it would happen.

She didn’t think Khara would get clean because she never got arrested. Not once.

For Liz it took multiple arrests and treatment centers to clean up.

“Getting in trouble saved my life,” she said.

Then, in 2010, Khara had her first son, Carson. she got pregnant with her second, David, and then stayed clean for good.

“She had her first child and here she was pregnant with David,” Liz said. “There was this moment where everything just changed.”

Pregnant, Khara packed up her things, loaded Carson into the car and drove to Texas to be with her sister. While she was there, Liz called to tell her about CLIMB Wyoming.

Since 1986, the nonprofit has helped 1,444 low-income single mothers across the state gain independence and overcome poverty by training and placing them in higher-paying careers. Women receive individual and group counseling and learn parenting and life skills in a 16-week program.

CLIMB expanded in 2004 and now includes six sites across the state. The Casper program has graduated 217 women, with a 90 percent graduation rate.

Less than half of the Casper graduates are employed when they apply. Of those who do have jobs, average monthly income is $1,105. Two years after graduation, that average is $2,550, according to most recent data.

This comes at a time when more women are raising kids as single moms, in a state that boasts the largest gender wage gap in the country.

In 2009, 34 percent of children in Wyoming were born to unmarried mothers, up from 29 percent in 2000, according to the 2012 Wyoming Kids Count Data Report. Women here earned 64 cents for every $1 earned by men working full time, year-round, in 2010, according to the National Women’s Law Center.

When selecting participants, Casper program director Lesha Thorvaldson said she looks for women who are truly ready to make changes in their lives. The program helps the women look at how the choices they’ve made have either helped or hurt them. they talk about their relationships, mental health, how they raise their children. some women apply two, three times before getting in.

“It’s looking at every single thing in their lives and picking out what can help them be successful in every aspect,” Lesha said.

Khara thought CLIMB could be her second chance. Before she had even submitted an application, she moved back to Casper.

In her first conversation with Lesha last October, Khara said she didn’t want to shame her boys by living in poverty. she didn’t want them to be embarrassed in the checkout line at the grocery store when they were old enough to know she was using food stamps.

She was motivated, determined.

“I knew instantly, she’s a keeper,” Lesha said.

Khara set five alarms to make sure she got up on time.

She never missed a day.

“Once she got back, I never had anything to worry about,” Liz said.

Liz ran errands and took Carson, now 20 months, and David, 8 months, to the doctor so Khara could focus on CLIMB. for the first time, Khara took her boys to day care.

In January, Khara spent two weeks in the McMurry Training Center classroom learning about welding. Then she was in the shop, welding and cutting metal, 36 hours a week for eight weeks. Khara and the other women met Wednesdays at the CLIMB office for counseling and classes in self-defense, domestic violence prevention, interviewing for a job, parenting, cooking and budgeting.

Training ended one Friday in March. Khara wanted to get right to work.

She interviewed for a job with Woodward Machine Corporation the following Monday and started work the same day.

She wakes at 6 a.m. these days, gets her boys up, changes diapers, puts “Sesame Street” on Netflix. The boys go to the sitter’s house and, from 7:30 a.m. to 5 p.m., Khara welds.

This is her way of life now.

“This is the way it should be,” she said.

Babette Curtis said she has her sister’s back.

On Wednesday, the Casper CLIMB program honored its mothers in a graduation, celebrating the end of 16 weeks. for Khara it also celebrated eight years.

Eight years since she started using, had her boys, got clean.

Dressed in black gowns and caps with red tassels, the women walked up to the podium one by one to receive their certificates. each was given the chance to speak.

But what to say?

Today, Khara has a career. She’s not on food stamps. she wants to buy a house and take her boys to Disney World and raise them to be good men. a couple of weeks ago, she opened college funds for them.

Khara stepped up, a smiling David in her arms. There were hugs, flowers, photographs.

She said her life has changed.

She said all that was left: “Thank you guys so much.”

“Thank you.”
